Registration Instructions

1) Coaches must fill out the information below to complete their initial registration ASAP.

2) Coaches must download and print the Roster/Waiver Form below and have parents/guardians fill out the form and coaches will email completed form to Contact@EliteFeet3v3.com, and also bring this form with you to the event.

Age Verification

Coaches are required to show Player's proof of age, at the registration table, the day of the event. This can be a printed copy of ID card, D/L, Birth Certificate, or any government issued ID, etc.

Team Registration

Coaches, in the TEAM ROSTER section below, please include the players First and Last Name, DOB, CURRENT Club Name (if player card holder), High School (If Applicable)
